Start with the layout, not the drill

Before you pick up a tool, decide exactly where the holder should go. A toilet paper holder is most comfortable when it’s easy to reach from the toilet without awkward twisting, and the centerline usually lands a little in front of the bowl’s front edge rather than beside your knee. For a solid backing, a proper screw into framing or masonry may be enough. If you don’t know what’s behind the tile, remove the existing holder carefully or inspect from a nearby outlet, switch plate, or unfinished side of the wall if possible.

Check the wall before you drill If you’re not sure whether the tile is over drywall, cement board, or masonry, verify the backing first. The anchor choice depends on what’s behind the tile, and the wrong fastener can either loosen quickly or crack the surface when tightened.

Also make sure the holder’s mounting hardware is compatible with tile thickness. Start by marking the hole locations carefully. A piece of painter’s tape can help the bit stay put at the start, and it also makes the mark easier to see. Once you’ve confirmed the location, begin with light pressure until the bit bites through the glazed surface. After that, you can continue slowly, still keeping the drill as steady as possible.

Heat is another enemy of clean holes. If the bit is running hot, pause and let it cool, especially with porcelain tile, which is harder than ceramic and less forgiving. A little water to control dust and cooling can help in some cases, but only if it won’t create a mess or interfere with the drill. Keep the area dry enough to work safely and avoid making the surface slippery.

When you reach the wall material behind the tile, switch your attention to the substrate. Stop and assess whether the anchor needs a smaller pilot hole, a different bit, or a different fastening approach entirely. The tile is only half the job; the backing is what keeps the holder from wobbling later.

Seal the holes and keep moisture out

Bathrooms bring a moisture issue that dry rooms don’t. Once you’ve drilled through tile, you’ve created a path where water can enter if the mounting points aren’t treated thoughtfully. That doesn’t mean you need to waterproof the entire wall, but the fastener penetrations should be sealed in a sensible way.

A small bead of bathroom-grade silicone or another appropriate sealant around the mounting points can help reduce moisture intrusion, especially near tubs or showers where the wall sees more splash.
